Welcome New Patients
We are excited to welcome you to our practice for your first appointment! To ensure we can provide you with the best possible care, please bring the following to your initial visit:
- A completed New Patient Registration Form (download PDF here).
- Any relevant medical history, including previous results, medical reports, or imaging.
- Your Medicare card.
- Your concession card(s) (if applicable).
- Any other information that might assist your doctor.
Before Your Appointment
To help us get you registered and ready for your consultation, we kindly ask that you arrive 15 minutes before your scheduled appointment time.
Billing Information
Please note that our practice operates as a private billing practice, and payment is required at the time of your consultation. We do offer considerations for patients holding Health Care Concession or Pension Concession Cards.
