New Patients

We look forward to welcoming you in the Practice for your first appointment.

To assist us, we would appreciate you bringing the following information with you:

• a completed New Patient Registration Form(download PDF & print)
• any relevant medical history
• previous results, medical reports or images
• your Medicare card
• your concession card/s (if relevant)
• any other information that would be useful to the doctor.

Also where possible, please arrive fifteen (15) minutes prior to your scheduled appointment time, so that we can register you on our system, ready for the Doctor.

Note: Our Practices are private billing and therefore, payment is required at the time of your consultation. We do however, take into consideration patients who hold Health Care Concession or Pension Concession Cards.
